Privacy Statement – Health Sciences Assurance Consulting, Inc.

Introduction

This notice sets for the policy of Health Sciences Assurance Consulting, Inc. (“HSAC”) with respect to the privacy of users (the “Privacy Policy”).

HSAC is committed to the privacy of our customers, prospective customers, and other third-parties, especially with respect to the Protected Health Information (PHI) or other personally identifiable information received by HSAC.  HSAC’s commitment to privacy extends to the operation and use of its website and online third-party platforms utilized by HSAC (collectively the “Website”) for the benefit of its customers and prospective customers.  This privacy policy is in addition to other privacy measures taken and notices provided by HSAC as required under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). 

Acceptance of Privacy Policy

Your acceptance of this Privacy Policy is based upon your use of the Website.  If one or more of the terms of this Privacy Policy are unacceptable your recourse is to leave our Website immediately.  Your continued use of the Website following the posting of changes to this Privacy Policy will mean that you accept those changes.

Personal Information We Collect and Use at This Website

We may collect personally identifiable information (PII) via your use of the Website should you voluntarily provide such information to us.  PII includes, but is not limited to, your phone number, name, age, address, and taxpayer identification number. If you encounter a screen or page that requests information you do not want to share with us, do not enter the information and do not proceed with that screen or page.

HSAC utilizes the services of By the Pixel for the collection and organization of information.  Information collected through the HSAC Portal will be housed through our health cloud system which is HIPPA compliant and will also be subject to the terms and conditions of our privacy policy.  You agree to your personal and financial information being transferred, stored, and processed  in accordance with our privacy policy.

Disclosure of Personal Information to Third Parties

HSAC will not sell, rent, license, or trade your PII with third parties for their own direct marketing use unless we receive your express consent to do so.  Unless you give us permission to do so, we will not share your PII with third parties other than as specified in this Privacy Policy.  

HSAC may share data with trusted partners to help perform statistical analysis, send you email or postal mail; or to provide customer support.  HSAC may also share data with university and college administrators of our customers, as well as legal, regulatory, audit and other professional advisors.  All such third parties are prohibited from using your PII except to provide these services to HSAC, and they are required to maintain the confidentiality of your information.

In addition, HSAC may share your PII if deemed necessary, in the sole and absolute discretion of HSAC, for the establishment, exercise, or defense of legal claims, whether in court proceedings or in an administrative or out-of-court procedure or as otherwise required by law.  The legal basis for this processing is our legitimate interest, namely the protection and assertion of our legal rights and the legal rights of our customers.

We restrict access to personal information collected about you at our Website to our employees, our affiliates’ employees, or others who need to know that information to provide services to you or are conducting our normal business operations. Your PHI and PII may be shared electronically with these parties or other authorized entities as permitted by state and federal law. We maintain appropriate physical, electronic, and procedural safeguards to protect your personal information.

Use of Email

We are committed to keeping your e-mail address confidential. We do not sell, rent, or lease our customer lists to third parties, and we will not provide your email address to any third-party individual, government agency, or company at any time unless strictly compelled to do so by law or as set forth in this Privacy Policy.

We will use your e-mail address solely to provide timely information related to the services provided by HSAC, and we will maintain the information you send via e-mail in accordance with applicable laws and regulations.

Use of Cookies or Other Data Collection Tools

A cookie is a small file placed onto your device that enables Website features and functionality. Any browser visiting our sites may receive cookies from us or cookies from third parties such as partners or service providers. Cookies may include either persistent cookies or session cookies. A persistent cookie may help us recognize you as an existing user, so it’s easier to return to the Website or interact with our services. A persistent cookie remains in your browser and will be recognized by the Website when you return to one of our sites or a partner site. Session cookies last only as long as the session (usually the current visit to a website or a browser session).

Children’s Privacy Protection

HSAC does not knowingly collect or retain personally identifiable information about persons under 13 years of age. Any person who provides their personal information to HSAC via the Website represents that they are 13 years of age or older. The Website does not knowingly solicit or collect personally identifiable information online from children under the age of 13 without prior verifiable parental consent.  If we learn that a child under the age of 13 (or such legally required higher age) has submitted personally identifiable information online without parental consent, we will take all reasonable measures to delete such information from our databases and to not use such information for any purpose (except where necessary to protect the safety of the child or others as required or allowed by law). 

Minors under 18 years of age may have the PII that they have provided to us through the Website deleted by contacting us at info@hsac.com and requesting deletion.  Please note that, while we make reasonable efforts to comply with such requests, deletion of your personal information does not ensure complete and comprehensive removal of that data from all our systems and back-up systems. 

Privacy Settings/Opt Out/Changes/Access

If you would like your PII removed from our database, changed or updated, you can contact us at info@hsac.com If requested, we will promptly verify and delete your account and you will no longer receive emails from HSAC. Your removal from the mailing list or database will not remove records of past transactions or delete information stored in our data backups and archives where we are required to keep your data for legitimate business or legal requirements. Data on past transactions and data stored in backups and archives will be deleted in the normal course of our business.  You have the right to access or, if required by applicable laws, receive a copy of your PII held by us by making a written request by sending an email to the contact email address below.

Website Directed to and Operated in the United States

This Website is only intended for, and directed to, customers and users located in the United States.  The servers are located in the United States and are subject to the applicable laws of the United States.  If you choose to visit the Website, you consent to the transfer, use and disclosure of information in accordance with this Privacy Statement and subject to such applicable laws.

Revisions to Privacy Policy

This Privacy Policy is current as of the Effective Date set forth above. However, HSAC may update this Privacy Policy from time to time as necessary. The terms that apply to you are those posted here on our website on the day you use our website. We solely advise you to print a copy for your records.
